Very Rare Wondrous Item (Mirror), Requires Attunement 

Macabre Reflection: While within 5 feet of a dead humanoid creature that is of creature size medium or small, you can use your action to capture their reflection in the mirror. Any creature looking into the mirror will see themselves with the form of the Captured Reflection

A corpse that has their reflection captured does not physically age as long as they are dead, but their time of death still increases for the purposes of resurrection spells.

Only one reflection can be captured in the Mirror at a single time, capturing another reflection will replace the preceding one. 

The reflection is further used in other of the Mirror’s features. 

Dead Appearance: If you are of creature size medium or small, you can use your action to gain the illusory appearance and voice of the Captured Reflection for 24 hours. The illusion is perfect and cannot be discerned without magical means. If the Captured Reflection has specific features that vastly differ from yours (A different number and shape of appendages, significantly different height, etc…) The illusion will appear as if you had those features as well, even when physically touched.. Your true form however is not changed, and you see yourself without the illusion unless looking into the mirror. 

The Captured Reflection in the Mirror is lost when you use this feature. 

Body Swap: When you use Macabre Reflection and have a Captured Reflection in the Mirror, you can choose to change the appearance of the corpse to that of the current Captured Reflection, before you replace it with the new one. 

Stolen Life: Once per long rest, you can use your action to force a creature within 5 feet of you that isn’t blinded to make a DC 13 Charisma saving throw. On a failure, the creature takes 3d10 psychic damage and they become silenced, unable to speak or cast spells with a verbal component for the duration of the effect. The affected creature can repeat the saving throw at the end of each of their turns, ending the effect on a success.
The Captured Reflection in the Mirror is lost when you use this feature, being replaced by the face of the creature affected by this feature, which cannot be used for features that require a Captured Reflection.
While the effect is active, and the affected creature is of CR equal to your level or lower, you can once per the duration of the effect transform into that creature as via the Polymorph spell, but keeping your mental ability scores, while still requiring concentration. This transformation can end in the same ways as the Polymorph spell, or if the affected creature dies, is incapacitated, or succeeds on this saving throw against the effect of Stolen Life.
